The form lists the date as June or July 2000, the location as Oakbank woods in East Calder near Livingston, and the animal as black and large, sitting in the centre of a tiny tree with its long black tail hanging down and curled. It did not move except for the swaying tail, and stayed for about ten seconds. He said another East Calder woman had seen it, and named a newspaper article as a reference.
A sheep's ribcage with wool still attached is not proof of anything, since foxes, dogs and natural death all leave such remains. The pin is placed on East Calder and is approximate.
